I don't know what is wrong, but I have a question, is that substrate just gravel? Also, what is the tank size and water parameters?

those are stress markings. Between the piles of poo, the lack of a working filter, and the small tank size (far too small for the pleco) It's a good bet that that water is a toxic nightmare. The pleco (and any other fish in there) are likely to be susceptible to a range of problems, from stress (as shown by the markings) to illness to death.

If you were the owner of the tank I would give you tips for improving the water conditions while stressing that even when properly maintained it would not be a healthy environment for a pleco. But you're watching it for someone else, so the situation may be different. How long are you watching it for? Are you willing to put any time/effort/money into improving things?

First of all, a common pleco doesn't have much of a life in a 2 ft tank when the pleco reaches one foot himself. But he looks to me as he is developing a bacterial problem. Another thing to check is the caves he's crawling into. If he hasn't enough room, he may be scraping his back trying to get in and back out.

It looks like i will have it for a long time i have emailed the owner but no reply have allready lost one of his fish through what i belive was stress due to water and the way in which they were transported. I am willing to put myh time and effot into helping them out money is the key issue at the momment barrely keeping myself healthly but any sujestions you have would be very helpfull. im a little unsure on the pleco being to big for the tank he is 8inchs end to end is that realy too big for a 2ft tank? thank you for your quick reply obviously this is a worrying issue

8 inches is MUCH too big for a 2 ft tank, to read on what common plecos need, click here

give the tank a good gravel vac, 40-50% water change, and get a filter for starters.

while it's great that you are here seeking advice for your inherited fish, without a bit of money to drop into that setup, you're not going to have much success.

I would rehome the pleco, empty the tank, and save up for a nice filter. while you are saving, you could research proper fish for that size aquarium. it will pay off in the longrun when you finally have a setup that works.
