I can't tell by the pictures, but if it looks like irregular shaped spots that are slightly indented, it could be the start of hole in the head.

That's kinda what I thought it might be. 2 jack dempseys (small), another angel, they could have been lip locking or something since ones a Male and the other a female. A couple small black skirts, 3 giant danios, a boat load of plecos, a ornate bichir, and a rainbow shark. All my fish get a long just fine and never had a problem with any of them, the rainbow shark doesn't even chase. The one jack Dempsey is a runt I'm assuming since he hasn't grown in over a year and my blue jack dempsey is about 4-5 inches long and very mellow.

No. Its white stuff under his mouth. He's a pretty aggressive angel fish and he has a female that's slightly bigger so I'm wondering if they could have been screwing around

Oh I see now, it looks like the result of lip locking. I wouldn't worry about it unless it gets larger.

Fresh water may help it clear up, I'd do some larger WC's than normal.

I would research the compatibility of those fish too, if you haven't already.

What size is this tank?...how is it filtered?

Thanks yeah I know about that stuff but everyone seems to get along good for now they have a lot of room. It's a 125 gallon tank. My blue jack dempsey is 5 or 6 inches long and hasn't tried to eat anyone yet so keeping my fingers crossed.

Keep a close eye on it for sure. Maybe take daily pics to see if it changes. Hopefully for the good. It could be scar tissue, my paired angelfish commonly have white at the tip of their lips, but in your pic, it's a bit low for lip locking. It could still be scar tissue from someone head butting him or something. I don't worry about scar tissue, it fades/heals on its own. If it's fuzzy at all, either some fungus set in or it's something bacterial. No other fish are showing this and no other symptoms with this angel?
