Hmmmm, the thing is that this seems to have all happened at once, and I've added nothing new for almost a year. The last thing I added was a type of styrofoam to prop up some rocks maybe a year ago....

It's a 90 gallon tank, and I always use water conditioner on every water change. I know it's supposed to remove chlorine and chloramines, but I'm unaware as to the heavy metal stuff you mentioned.
