If it is stressed a fish is most likely nipping...
If it is stressed a fish is most likely nipping on it. It it is acting normal, it most likely injured itself. Check the driftwood and rock in your tank to see potential ways it could of got hurt.

I'm pretty sure ick is when they get white dots and not spots that look damaged. If you look carefully in the front, you can see it. It could be fungus but there are many other fish in the community...
