No not all flowerhorns are like that, that is a rare one and probably has been fed with too much of special foods or even hormones.
flowerhorns are so popular in east Asia and there are many kinds of them, some kinds like kamfa have big head humps.
also they produce some special ones with selective breeding and such things to make bigger hump and better coloration and pearls.

lol, i knew i saw that somewhere. aquagiant here (asian fish store) has about a dozen of those posters around the store, theres some for saltwater tangs, african cichlids, etc.

Its a natural part but that one may have been fed to alter it as Blu-ray said.
It is very disturbing though!
