View Full Version : Standard Form for Illness Issues/Other Problems

Is there a form or stickied post where folks can copy and insert their parameters? I keep seeing requests for help and honest efforts to help the posters, with always (intelligently) the requests for data to help the poster. I'm thinking that it could save both the poster and responder time. I've noticed that experienced users typically do this, but a newbie isn't going to think of all these things. Just a thought.

Ammonia level <insert ppm here>
Nitrites level <insert ppm here>
Nitrates level <insert ppm here>
Water hardness <insert dH here>
<Specify other chemical levels that might be relevent here>
Volume of tank <insert #gallons and/or #litres here>
Size of tank <insert length x height x width here in inches or cm>
Filtration system <insert type and volume filtered>
Temperature <insert current temp here>
Age of tank <insert age of tank in years or months here>
If a new tank when was it cycled and how?
Number and amount of water changes weekly
Age of fish <if known>
Other relevant information (just installed new plants, just changed filtration systems, changed food type, just installed a CO2 system, added new residents to the tank (type of resident and number), have started xyz medication, note other changes since disease was noticed, etc.)

I did not see anything like that form stickied in the Disease forum and think the form would be very good to have in that section to help people. thumbs2:

Thanks, Dave66 thought it was a good idea as well, and including some links to explain some of the different chems/terminology. Probably it could be stickied to several areas of the forum.

Thanks, Patriot. When I get a chance I want to also update some of the items with links to articles to help folks out more. I will add types of fish.
